Cardiac Risk Profile Clinic
Cholesterol Screening for State Employees, Retirees, and their Families
Event: Cardiac Risk Profile Clinics
When: Tuesday, July 22, 2008, from 7:00 a.m. to 8:45 a.m.
Where: Idaho Department of Environmental Quality, First Floor, Conference Room B, 1410 N. Hilton,
Boise
What: includes a blood pressure check and a blood draw for a cardiac risk
profile which can assist you in identifying potential risk factors for cardiovascular
disease. The profile includes readings for HDL, LDL, cholesterol/HDL ratio, glucose, triglycerides, and more. You are encouraged to share your results with
your doctor.
For: State employees, spouses, retirees, and family members
Instructions: This is a fasting blood test: no food for 12 hours prior to the test
and no alcohol for 48 hours prior to the test. No doctor’s order is needed for
this test.
Cost: only $22; payment is due at the clinic. This is not reimbursable under
the State of Idaho’s health insurance plan but may be submitted to your Flexible Spending Plan.
Register: You must register to participate; go to My Sign Up and select a date and time. Complete the Client Data Form and
bring it with you to the clinic.
Additional Tests: Big Savings!! Additional blood tests are available at a
savings through these clinics with a Doctor’s Order. If you need a PSA test or a thyroid screen, this is an affordable option. The additional tests will
be billed to your insurance provider and are eligible for insurance coverage.
Review the list of additional blood tests available. Bring your Doctor’s Order with
you to the clinic along with a completed patient information form.

Event: Results & Feedback Session with Janet Coe, RN, Saint Alphonsus
Regional Medical Center
When: Wednesday, July 30th, 2008, 12:00 noon to 1:00 p.m.
Where: Idaho Department of Environmental Quality, Conference Rooms A & B, 1410 N. Hilton, Boise
What: You will receive your results in a sealed envelope. There will be a brief
presentation on the CRP profile, how to interpret your results, and a question and
answer period.
For: CRP Clinic participants and any interested individual with previous clinic
results they wish to better understand
Cost: none
